Pope Francis has called for an extraordinary Synod of Bishops, with a topic of Pastoral Challenges to the Family in the Context of Evangelization. The preparatory document for that Synod included a questionnaire. If you would like to provide input to the Bishops in advance of that Synod, you are invited to do so using that questionnaire. The time frame is narrow, and responses must be returned by Sunday, December 1st. There are several options:

For those desiring the simplicity of an online survey, that is not available from the U.S. Bishops. It is possible to use the survey provided by the Catholic Organizations for Renewal, results of which will be sent to directly to the USCCB. (Use of this form does not constitute approval of the sponsoring organizations.)
